Please find below some information with regard to privacy and consent. When coming to the appointment, we require you sign a copy of the consent. Should you have any queries, please do not hesitate to let us know. 

Information Collection & Purpose of Collecting & Holding Information

During all your sessions with the psychiatrist, he/she will collect and record personal information disclosed from you that is relevant to your assessment, diagnosis & subsequent treatment. This is necessary as it allows the psychiatrist to provide a relevant and informed medical service for you.

All relevant information is retained through summarised assessment and ongoing treatment notes, which are entered into an electronic medical records system – only accessible by staff working at this practice. This is separate to myHealth Record.

The clinical information recorded by the psychiatrist will only be shared with your GP or the referring specialist. Please see the exceptions below.

If you were referred through Workers’ Compensation, we also write a summarised assessment letter to them.

Access to your Information:

At any stage, you are entitled to access the information that has been kept on file under your name, unless the relevant legislation states otherwise.

For children/young adolescents under the age of 14/15, their parent/legal guardian are also entitled to access their information if required. However, young adults who are 16 or over and have the capacity for informed consent are able to select who can have access to their information. This is following a discussion with the psychiatrist.

Confidentiality

All personal information gathered by the psychiatrist during the provision of the psychiatry service will remain confidential & secure within an electronic medical record with the exception of:

  • Record is being subpoenaed by the court or;
  • If the failure to disclose information discussed places either yourself or another person/s at serious, imminent danger/risk (Mandatory Reporting or;
  • Your prior approval has been obtained, following Request for a written report to a third party agency or professional (e.g. lawyer/insurance)
  • To an alternate medical professional
    • Either following signed consent from you or
    • Conversation during an appointment
    • Discuss material with another person directly involved (e.g. parent/guardian)

Fees

We are a private practice – therefore, we are not a bulk-billing practice. The Medicare rebate only applies if you have a valid GP referral. It is your responsibility to ensure that there is a valid referral for your appointment (usually GP referrals last for 12 months & specialist doctors last for 3 months).

Private Health Funds do not apply to psychiatry consultations.

Cancellation Policy

Should you be unable to attend/cancel/postpone appointment, please provide us with at least 24-hours notice. Otherwise, you may be charged the cost of your sessions.
